Overview
We are seeking a dedicated and compassionate Home Health Registered Nurse to provide high-quality nursing care to patients in their homes. The Home Health Registered Nurse will work collaboratively with interdisciplinary teams to deliver comprehensive care that promotes independence and well-being.
American Family Health Services Inc is looking for Registered Nurses that will do home health visits on a per visit basis or full time. Tasks may include Admission of patients into home health, Re-certification of care, Resumption of care and Supervisory visits and Regular RN visits, Wound care, IV infusion and Quality Assurance.
Responsibilities
The RN will assess and chart observations of the patient’s condition at each visit, assess vital signs and blood sugar, dress or redress wounds and assess the healing progress, administering at-home intravenous (IV) infusion, and perform blood draw for lab work. The Nurse will provide education to patients and families on proper home health care procedures and strategies each visit. The RN will monitor the effectiveness of medications, filling medication box and teaching. The RN will coordinate care with other nurses, Physicians, Physical Therapists, Occupational Therapists, and all other individuals in the patient’s care plan. The RN will act as case manager, write orders and communications, and progress reports.
Requirements
- Current Texas State Board of nursing license - At least one-year of clinical nursing experience required - One year Home Health Care Experience is a plus but not required. The clinician must have reliable transportation with a current valid driver’s license, current auto insurance, current CPR, TB screen done in less than one year. The nurse must have excellent interpersonal skills, strong organizational skills and be detail oriented.

Recertification: $ 80.00 per visit
