Position Details
- Location: Lowell, MA
- Duration: 13 weeks to start, with the likelihood of extension
- Setting: Psychiatric/Behavioral
- Schedule: Monday - Friday, 8am to 4pm, with a standing 12pm-8pm shift. Includes rotating weekends
- Salary: $28/hr
- Provide case management services as part of a multidisciplinary team
- Develop treatment plans with consumers
- Act as a primary case manager to consumers in accordance with Eliot’s policies and PACT standards
- Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner
- Assess clients' functions, strengths, and needs
- Develop therapeutic connections with each client and provide clinical interventions
- Provide problem-solving supports and interventions to clients
- Caseload is 5-8 clients
- Valid driver's license
- Reliable vehicle, as you will be asked to take clients out into the community
- 1 years of prior experience working with adults having severe and persistent mental illness
