The Licensed Social Worker - Complex Case Unit - Complex Case Unit - Complex Case Unit collaborates with themulti-disciplinary team to provide input in the development of the plan of carefor those clients requiring social work intervention. The Licensed Social Worker - Complex Case Unit - Complex Case Unit - Complex Case Unit will providesupport, advocacy, and resources to individuals and families facing variouschallenges. This role involves conducting assessments, developing care plans,coordinating services, and collaborating with interdisciplinary teams topromote overall well-being.
Minimum Requirements:
- Bachelors or Master’s Degree in Social Work (MSW) from anaccredited institution required
- Current state licensure as a Licensed Social Worker - Complex Case Unit - Complex Case Unit - Complex Case Unit (LSW)required or current state registration as a Registered Social Worker (RSW)depending on state in which they will be working.
- Minimum of one year of social work experience preferred
- Complies with all relevant professional standards ofpractice
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Must be at least 18 years of age
