Job Description

Job Title : Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty

License : Must have a valid license in hand

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase EMR, used within the last 6 months, or completion of a retraining course
Reliable transportation, a current driver's license, and agency-required liability insurance
Must be willing to travel up to 75 miles from the home clinic
Comfortable with daily travel to patient homes and periodic office meetings

Job Description :

Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.

Be responsible to the Clinical Manager and directly supervise and evaluate Home Health Aides and LVNs / LPNs.

Expected to manage an average of 5-8 patients per day.

Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.

Complete all documentation within 24 hours of the appointment with the patient.

Meet weekly productivity points equating to 40 hours per week; assigned between 5-10 patients per day equating to roughly 6-8 points.

Participate in floating to nearby clinics if necessary to pick up additional assignments.

Local candidates accepted at the same rate, but must confirm driving capability and insurance.

Responsible for handling every patient on the schedule regardless of their location in comparison to previous appointments.
