Job Title : Registered Nurse I

Job Specialty : Home Health

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : Not specified

Experience : Minimum 1 full year of adult home health experience or 1 full year of hospice experience

License : Must have license in hand; pending license not accepted

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase (HCHB) and recent use within the last 6 months
Completed Home Health Checklist
Reliable transportation, driver's license, and auto insurance

Job Description :

Provides, coordinates, and directs home nursing care in accordance with physicians' orders and agency policies

Responsible to the Clinical Manager, directly supervising and evaluating Home Health Aides and Licensed Practical / Vocational Nurses

Conduct home visits and periodic office meetings

Assigned 5-10 patients per day equating to approximately 6-8 points

Engages in various types of cases including Start of Care, Wound Care, Follow-up Visits, etc.

Must travel up to 75 miles from the home clinic; mileage reimbursed at IRS Standard rate

Expected to complete documentation within 24 hours of patient appointments

Conversion of 30 points per week equates to 40 hours per week work expectation

Contractors may float to a nearby clinic to pick up additional hours if patient census is low

Weekend rotations and on-call duties vary by location and must be discussed during the interview

Familiarity with OASIS documentation is essential

Comfortable with driving between patient locations and managing a broad territory covering multiple areas

Please confirm understanding of the following :

This role involves daily travel to patients' homes and weekly meetings at the clinic

All scheduled patients must be accepted, even if distances are substantial

There are no guaranteed hours with this contract; productivity expectations must be met
