Responsibilities
- Demonstrates/models consistent use of positive and proactive behavior management techniques.
- Provides ongoing support to people by assisting in the development and implementation of all formal and informal support plans and demonstrated knowledge and understanding of specific supports and approaches for each individual.
- Consistently records necessary documentation in each individuals progress notes, data sheets, medication charting, incident reports, daily staff log in a timely and legible manner.
- Ensures that consumer’s routine daily tasks (grooming, laundry, room cleaning, and household chores) are completed as necessary and supervises and provides training and assistance as needed to assure tasks are adequately completed.
- Encourages consumer participation in a variety of recreational and leisure activities in the community by suggesting ideas, assisting in planning and participating with the consumers in the activities.
- Assist in the development and follow through of individual support plans by providing input, consistent implementation and charting of program and health plans and other duties as requested and time permits.
- Properly follow medication administration protocol and administers medication without errors.
- Maintains a valid driving license and good driving record and is willing to use personal vehicles for transport when necessary
- Assists with scheduling and/or accompanying consumers to medical appointments and assures appropriate communication and follow up.
