Amber Court of Westbury is seeking a dynamic Case Manager.
Case Manager Essential Job Responsibilities:
- Care Coordination: Acting as the central point of contact for residents, families, and healthcare providers to coordinate services for residents served.
- Assessments and Care Planning: Conducting initial and periodic residential interviews to implement, and update documentation based on the physical, emotional, and social needs of residents served.
- Monitor and Evaluate: Continuously monitoring the health and well-being of residents, while adjusting documentation to reflect changes in condition.
- Advocacy and Support: Empowering residents to voice their needs and ensuring they receive appropriate, satisfactory care. Collaborating with families and providers.
- Resource Coordination: Connecting residents with community services, such as specialized care, social services, and or medical services, to enhance their quality of life.
- Crisis Management: Responding effectively to emergencies, including sudden health issues, family crises, or changes in lifestyle.
- Regulatory Compliance and Documentation: Ensuring all documentation is accurate, and and compliant with state and federal regulations.
