Registered Nurse – Home Health
Location: Medford, OR
Duration: 13 Weeks Contract
Start Date: 11/24/2025
Shift Timings: Day 5x8-Hour (08:00 – 16:30)
Pay: $2406.00/week
Job Description:
- Provide skilled nursing care to adult patients in their homes across Medford and surrounding areas.
- Conduct comprehensive assessments, including admissions, resumptions of care, recertifications, and discharges.
- Deliver wound care, IV therapy, and medication management according to physician orders and agency protocols.
- Document all care accurately in Epic/OASIS and ensure compliance with state, federal, and agency regulations.
- Collaborate with interdisciplinary teams to create and implement individualized patient care plans.
- Educate patients and caregivers on disease management, medications, and home safety practices.
- Manage daily visit schedules efficiently while maintaining excellent patient satisfaction and outcomes.
Requirements:
- Minimum 1 year of recent Skilled Home Health RN experience – Required
- Active RN License: Oregon or Compact – Required
- Certifications: BLS – Required
- Charting: Epic and OASIS – Required
- Skills Required:
- Admission Assessment
- Start of Care / Resumption of Care / Recertification / Discharge
- Wound Care and Medication Administration
- Infection Control Precautions
- Pain Assessment & Management
- Case Management (preferred)
- Must have reliable transportation – daily travel expected within 50 mile radius (mileage reimbursed at IRS rate)
- First-time travelers accepted
- COVID vaccine 1 booster required
