Registered Nurse – Home Health
Location: Medford, OR
Duration: 13 Weeks Contract
Start Date: 11/24/2025
Shift Timings: Day 5x8-Hour (08:00 – 16:30)
Pay: $2406.00/week

Job Description:

• Provide skilled nursing care to adult patients in their homes across Medford and surrounding areas.
• Conduct comprehensive assessments, including admissions, resumptions of care, recertifications, and discharges.
• Deliver wound care, IV therapy, and medication management according to physician orders and agency protocols.
• Document all care accurately in Epic/OASIS and ensure compliance with state, federal, and agency regulations.
• Collaborate with interdisciplinary teams to create and implement individualized patient care plans.
• Educate patients and caregivers on disease management, medications, and home safety practices.
• Manage daily visit schedules efficiently while maintaining excellent patient satisfaction and outcomes.

Requirements:

• Minimum 1 year of recent Skilled Home Health RN experience – Required
• Active RN License: Oregon or Compact – Required
• Certifications: BLS – Required
• Charting: Epic and OASIS – Required
• Skills Required:
• Admission Assessment
• Start of Care / Resumption of Care / Recertification / Discharge
• Wound Care and Medication Administration
• Infection Control Precautions
• Pain Assessment & Management
• Case Management (preferred)
• Must have reliable transportation – daily travel expected within 50 mile radius (mileage reimbursed at IRS rate)
• First-time travelers accepted
• COVID vaccine 1 booster required
