Key Responsibilities:
- Conduct housing assessments and eligibility screenings
- Develop Housing Stabilization Plans (HSP)
- Assist with housing applications and tenancy support
- Coordinate with MCOs, Medicaid, and community providers
Qualifications:
- High School Diploma required; Bachelor’s preferred
- Experience in housing, case management, Medicaid, or social services
- Strong documentation and communication skills
