Role Description The Home Health LVN provides direct nursing care to patients in their homes, supporting the implementation of individualized care plans under the supervision of a registered nurse and/or physician. Day-to-day responsibilities include performing assessments, administering medications and treatments, monitoring vital signs and symptoms, documenting clinical findings, and communicating changes in patient status to the care team. The LVN coordinates with interdisciplinary team members, educates patients and caregivers on disease management and home care procedures, and ensures adherence to safety, infection control, and regulatory standards. This is a full-time, hybrid role based in Dallas, TX, with a combination of field visits within the North Dallas area and some work-from-home for documentation, care coordination, and virtual communication.

Qualifications

• Current Licensed Vocational/Practical Nurse (LVN/LPN) license in the state of Texas, with good standing and compliance with all regulatory requirements.
• Strong nursing and clinical skills, including assessment, medication administration, wound care, and hands-on patient care in a variety of home settings.
• Knowledge of home care practices, home health regulations, and best practices for delivering safe, effective care in the patient’s residence.
• Solid understanding of basic medicine and chronic disease management, with the ability to follow provider orders and recognize changes in patient conditions.
• Effective communication, documentation, and time-management skills, with proficiency using electronic health records and mobile technology.
• Prior experience in home health, hospice, or community-based care preferred, along with familiarity working within an interdisciplinary care team.
• Valid driver’s license, reliable transportation, and willingness to travel within the North Dallas service area.
• Ability to work independently, demonstrate sound clinical judgment, and provide compassionate, culturally sensitive care to diverse patient populations.
