Caseload: 50–60 clients/month
Location: Seattle, WA (field-based; community-based visits-meet the client where they are)
Schedule: Monday–Friday, 10:00 AM–6:00 PM

About the Role
We are seeking a dedicated and compassionate Care Coordinator to join our team and make a meaningful difference in the lives of individuals across King and Pierce Counties. This role is ideal for someone who is passionate about supporting vulnerable populations, enjoys building relationships, and thrives in a dynamic, client-centered environment.
As a Care Coordinator, you will play a vital role in helping clients access essential medical, behavioral health, housing, and community resources. You will support clients in navigating complex systems, advocate for their needs, and empower them to take an active role in their health and well-being.
What You’ll Do
- Conduct outreach to new clients and provide an overview of Health Home program benefits.
- Complete comprehensive health assessments in coordination with primary care and interdisciplinary teams.
- Develop, update, and monitor individualized Health Action Plans (HAPs) focused on client goals.
- Connect clients to medical care, mental health services, housing support, crisis resources, and social programs.
- Advocate for clients by helping them navigate healthcare systems and community-based supports.
- Provide education and tools that enhance client and family engagement.
- Assist with entitlement applications and connect clients to community events and local resources.
- Track hospitalizations and coordinate post-discharge follow-up to ensure continuity of care.
- Promote health literacy and support clients in becoming active participants in their care journey.
Minimum Qualifications
Candidates must meet one of the following:
- Bachelor’s or Master’s degree in social work, psychology, behavioral sciences, human services, or related fields,
OR
- Current licensure as an RN, ARNP, LPN, Psychiatric Nurse, Psychiatrist, PA, Clinical Psychologist, Mental Health Counselor, Agency-Affiliated Counselor, Marriage and Family Therapist, or Certified Chemical Dependency Professional,
OR
- Certified Medical Assistant with an Associate Degree, or IHS-Certified Community Health Representative (CHR).
Additional Requirements
- Must be 21 years or older.
- Ability to travel throughout King and/or Pierce Counties for in-person client visits.
- Reliable personal vehicle, valid WA State Driver’s License, and current auto insurance.
- Must pass state/federal background checks.
- Mileage reimbursement is already included in the hourly rate.
Preferred Skills & Competencies
- Experience in clinical or direct patient care (Required).
- Exceptional communication and customer service skills.
- Strong organizational skills and the ability to manage time efficiently.
- Ability to work independently and within a team.
- Comfortable using technology and adaptable to learning new software.
- Knowledge of HIPAA and commitment to confidentiality.
