Client Care Coordinator responsibilities include:

  • Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handling diverse case assignments, develop / update service plans , review/document case progress
  • Starting home care jobs; introducing home care team to family
  • Manage assessments for non-medical care
  • Coordinate community-based care agencies (hospice, VNA, PT, OT)
  • Act as a liaison with family
  • Helping clients achieve wellness and autonomy
  • Document visits against care plan and discharge goals
  • Motivate caregivers; appreciate them; provide on-the-spot coaching

Responsibilities

  • Manage, coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handle case assignments, draft and maintain service plans, activity schedules for caregivers, review case progress and determine case closure
  • Orient caregivers to new cases
  • Assist with coordinating other services to assist with living independently (i.e chair lift, ramp, rides to appointments, peapod, etc)
  • Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop effective working relations and cooperate with medical and non-medical team throughout the entire case management process
  • Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
  • Record case information, complete accurately all necessary forms and produce statistical reports
  • Promote quality and cost-effective interventions and outcomes
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Manage on-call 1 week a month including weekends.

Requirements

  • Proven love for Seniors and the communities they live in!
  • Enthusiasm for managing a diverse number of cases and proven ability to communicate successfully with families, nurses, social workers, administrators, and caregivers
  • Proven working experience in home care or related field
  • Effective and dynamic communication skills
  • Excellent organizational and time management skills
  • Eagerness to learn and be part of a growing management team
  • Out of the box thinking and ability to multi-task
  • Compassionate with teamwork skills

Job Type: Full-time

Pay: $40,000.00 - $50,000.00 per year
