- Conduct outreach to caregivers identified as ready for enrollment into ACC services.
- Assist in obtaining and tracking the submission of medical information forms.
- Serve as a liaison to connect patients and caregivers with appropriate services within and outside of ACC.
- Track and document attendance schedules and daily absences at day centers.
- Provide backup support to the Intake Coordinator.
- Monitor and document progress of caregivers and patients through the admissions process until services begin.
- Schedule and coordinate tours of ACC day centers.
- Maintain accurate caregiver and patient data; update the electronic health record (EHR) as needed.
- Support administrative projects including data entry, reporting, case management, and filing.
- Travel to day centers to serve as a patient aide, ensuring compliance with required staff-to-patient ratios.
- Strong organizational and time-management skills; ability to work independently and prioritize tasks.
- Proficiency in Microsoft Office Suite, especially Word, Outlook, and Excel.
- Excellent communication and interpersonal skills.
- Customer service oriented, with the ability to build positive relationships with patients, caregivers, and internal teams.
Required:
- High school diploma or equivalent.
- At least one (1) year of professional experience in administrative support.
- Experience in administrative roles related to healthcare or patient services.
- Shared office space with moderate noise and limited workspace.
- May be exposed to offensive odors.
- Interaction with up to 30 people daily across a department of approximately 70 employees.
- Extended hours may be required in emergency situations.
- Must be AHCA/DOEA eligible.
