Job Title: Patient Navigator
POSITION SUMMARY
Patient Navigator provide a wide variety of front desk administrative and clerical support as needed. This includes serving as an integral component of the patient check in and check out process, coordinating / scheduling appointments and transportation, receipting calls, as well as compiling medical records & charts. The MOA maintains a positive and professional image when interacting with patient population, their family, physicians, employees and general public. Completes various tasks as assigned.
ESSENTIAL JOB FUNCTIONS : *Not an exhaustive list of duties*
- Administrative: Greet patients and reception of incoming calls. Execute check-in and check-out process. Receives and screens communications, transmits detailed information, answers questions, takes accurate messages, and keeps staff informed of communications received. Orders and maintains office supplies. Manage waiting room area: ensure lobby adheres to AVS continuity binder standards.
- Scheduling: Receives and accurately processes referrals ensuring that all required information is accurately provided. Coordinates and maintain the electronic appointment calendar using discretion in scheduling and rescheduling as appropriate with EMR. Schedules and assists with meetings, education classes, and special initiatives. Coordinates patient transportation as requested.
- Medical Records Request: Complete medical record requests in a timely manner pursuant to SOP’s. Ensures that records are maintained in an accurate manner while maintaining confidentiality according to departmental policies and procedures.
- Data Entry: Accurately enters patient information into computer database and updates as required to include creating patient EMR with demographics, routinely solicits for up-to-date patient information.
- Billing: Proficiently verifies patient insurance information by methods driven by SOP’s, updates appropriately, and collects cost-share/co-pay/patient balances.
- Customer Service: Presents and conducts self in a professional manner. Provides support to coworkers and customers while demonstrating excellence service by building positive interpersonal relations to ensure productive and positive relationships. Develops a deep understanding of patient perspectives and needs and works to meet their expectations. Sustains and maintains patient confidentiality pursuant to HIPAA.
- Communication/Teamwork: Communicates in a compassionate and professional manner to patients, families and visitors. Demonstrates professional interpersonal skills and cooperates with other members of department. Demonstrates commitment to teamwork and interacts harmoniously with others. Supports new orienteers in learning department routines.
- Critical Thinking and Organization: Utilizes available resources. Recognizes workflow errors/problems/areas if improvements and submits to supervisor as required. Demonstrates effective priority setting and organization skills to promote department flow and efficiency. Maintains neat and organized work area. Anticipates and prioritizes workflow and adjusts work assignments to prevent delays and meet work flow demands, Assists with problem resolutions.
- Initiative: Demonstrates knowledge and support of quality improvement goals. Participates in department, facility and organizational initiatives or committees as assigned.
- Development : Able to grow, develop, and positively implement professional/constructive criticism. Employee should demonstration a willingness to self-improve.
- Reliability and Attendance : Employee is expected to arrive on-time pursuant to schedule as well as meet schedule demand as required.
- Performs other related duties as required.
- ESSENTIAL JOB QUALIFICATIONS:
- Education: High School Diploma or equivalent required. Medical Certification preferred (MA,PCA).
- Experience: Minimum of two years secretarial/clerical experience required. Healthcare experience is preferred.
- Licensure/Certification: N/A
- Other Qualifications: Must be detail oriented, able to organize work, set priorities, and remain comfortable with rapid changes in priorities. Must be able to work independently in a fast-paced environment with limited direct supervision. Must be proficient in the use of computers and software programs. Knowledge of medical terminology preferred.
- Attendance: Regular punctual attendance as scheduled is a requirement of this position.
- English Language: Must be able to read, write, and speak English.
- Employee must be able to remain in stationary position for 75% of time, person needs to occasionally move about inside office to access file cabinets, office machinery etc. Constantly operates computer and other office productive machinery such as printer, fax, scanner, etc. Occasional requirement to position self to stoop, kneel, and or crouch when adjusting equipment or stocking supplies. Person must frequently communicate with patients, employees and physicians to further ensure coordination of care. Must be able to lift weight of 15 pounds or less.
