Overview
We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients with diverse needs. Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately within electronic health record (EHR) systems.
- Support medication administration following prescribed schedules while maintaining compliance with HIPAA and home healthcare regulations.
- Provide specialized care for patients with dementia, Alzheimer’s disease, or other cognitive impairments, including memory care strategies and behavioral management.
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe and hygienic environment.
- Support patients during end-of-life or hospice care with compassion and respect for their dignity and comfort.
- Document medical records thoroughly and accurately to ensure continuity of care and compliance with legal standards.
- Collaborate effectively with nursing staff and other healthcare professionals to deliver comprehensive patient care tailored to individual needs.
Experience
- Prior experience working as a Home Health Aide or in assisted living facilities is highly desirable.
- Demonstrated proficiency in patient observation, medical terminology, and basic life support (BLS) including CPR certification.
- Experience caring for geriatric patients or individuals with disabilities such as developmental disabilities or chronic illnesses like dementia or Alzheimer’s disease.
- Familiarity with EMR (Electronic Medical Records) systems and home health agency operations.
- Knowledge of home & community care practices, hospice protocols, catheter care, medication administration, and vital signs monitoring.
- Previous experience working in long-term care settings such as nursing homes or residential facilities is advantageous.
- Ability to lift heavy objects safely and perform physically demanding tasks required for caregiving responsibilities.
