Nurse Assessor
Job Summary:
Join our team as a Nurse Assessor at a leading healthcare agency. As a Nurse Assessor, you will play a crucial role in assessing and evaluating patients' health conditions, developing care plans, and ensuring the delivery of high-quality healthcare services. Duties:
- Conduct comprehensive assessments of patients' physical and mental health status inside of patients' homes.
- Develop individualized care plans based on assessment findings and collaborate with healthcare teams to implement them effectively.
- Utilize your expertise in areas such as trauma medicine, primary care, occupational health, dementia care, and behavioral health to provide specialized care.
- Educate patients and their families on treatment plans, medication management, and preventive healthcare measures.
- Requires driving to multiple patient's homes daily.
- Homecare Homebase experience preferred
Qualifications:
- Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing (MSN) preferred.
- Active Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of experience in a clinical setting, preferably in a teaching hospital or similar environment.
- Proficiency in conducting assessments, developing care plans, and implementing evidence-based practices.
- Strong communication skills and the ability to work effectively within interdisciplinary teams.
- Experience in areas such as trauma medicine, occupational health, home health, home care, and dementia care is highly desirable.
