What You'll Do

• Deliver in-home physical therapy visits across Oahu
• Perform evaluations, reassessments, and fall-risk screenings
• Track progress carefully and keep the care team in the loop
  
  
  

What You Bring

• Active Hawaii license as PT or PTA
• At least 1 year of clinical experience (home health a plus)
• Sharp documentation habits and clear communication
• A valid driver's license and dependable transportation
