Home Health Physical Therapist (PT) / Physical Therapist Assistant (PTA) – Per Diem / PRN
Pay
PT: $80 per visit
PTA: $55–$60 per visit
Location: O‘ahu, HI (reliable transportation required)
Schedule: Flexible / Per Diem (PRN) – Build Your Own Caseload

We are currently hiring licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) who are passionate about providing high-quality, patient-centered home health care.

Responsibilities
- Provide skilled in-home physical therapy services
- Complete evaluations, reassessments, and fall risk assessments (PT)
- Implement individualized treatment plans to improve mobility, balance, strength, and safety
- Educate patients and caregivers on home exercise programs and fall prevention
- Document accurately and communicate patient progress with the interdisciplinary care team
- Maintain compliance with Medicare home health documentation requirements
Requirements
- Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
- Minimum one year of clinical experience (home health experience preferred)
- Strong documentation, communication, and time-management skills
- Ability to work independently in the field
- Valid driver's license and reliable transportation
- Passion for serving seniors, kupuna, and local families
