Responsibilities
Provide high-quality, patient-centered care to clients in their homes, as part of our skilled home health care services
Conduct patient assessments, and implement interventions as needed
Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
Perform wound care, and provide other skilled nursing interventions as ordered by physicians
Document patient care and progress accurately and thoroughly
Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
