We are looking for dependable and clinically skilled nurses who are comfortable working independently in a home-care setting. Flexible availability and drivers are preferred.

DETAILS

Flexible Scheduling

• Hourly and per diem opportunities available
• Initial visits available
• Minimum case length: 4 hours
• Flexible scheduling based on your availability
• Opportunities available throughout Suffolk County

Responsibilities

• Provide skilled nursing care in the client's home
• Complete nursing assessments and monitor changes in client condition
• Develop and follow individualized care plans
• Coordinate care with clients, families, caregivers, and other members of the healthcare team
• Provide education and support to clients and their families
• Document patient care accurately and timely
• Recognize changes in condition and communicate concerns appropriately
• Provide compassionate, patient-centered care
  
  
  

Qualifications

• Valid New York State Registered Nurse (RN) license required
• Home care experience preferred
• Strong clinical assessment and communication skills
• Comfortable working independently in a home-care environment
• Reliable, professional, and patient-centered
• Must be authorized to work in the U.S.
• Must have updated medicals/vaccinations or be willing to obtain them
• Valid driver's license and reliable transportation preferred
• Completed drug screen or completed Habituation Form
• Must successfully complete the Alvita Care onboarding process before accepting cases
