Requirements Robert Wood Johnson- Monmouth Medical Center Southern Campus- Kimball needs an Anesthesiologists for coverage that can start ASAP- beginning of December. **ONLY submit your provider if they can start in December!** 3 month committment to start with possible extension. Please list any anticipated procedures/surgeries the provider will be required to perform and anticipated volumes: Pediatric Dental, General surgery, ortho, Urology Description of Work Assignment: 7a-5p, guaranteed hours per CAO, CMO request Breakdown of Clinic Days/Sessions and/or OR Days Monday through Friday Schedule of Days/Times Needed to Work/ Shift Hours: 7a-5p, must stay in house even if no cases until call team starts at 5p. On call? No. Patient Load: Less than 150 cases a month Board Status Requirement (BC/BE) Board Certified Is there additional support staff? 1 FTE, Joint ventures, per diem, Monmouth Main staff Is the provider working solo or with other providers? If so, whom? NA Are there Advanced Practice Providers? If yes, is the locums provider responsible for them? Physician model. There are no CRNA's. Attending physican solo cases. Trauma Level of Work Location No trauma, no heads, no hearts EMR System? EPIC Is the Clinic EMR Different from the Hospital EMR? No List of individuals interviewing candidate: Orlando Gopez, Mike Cavallo, Onofrio Patafio MD Time Sheet Approver (once physician is hired) Orlando Gopez VP Anesthesiology Are we able to grant full, temporary, or emergency privileges? If so, what dates does the committee run? Emergency Privileges through MSO
