Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health by taking vital signs such as blood pressure, pulse, temperature, and respiration; report changes to healthcare professionals promptly
- Support medication administration following proper protocols and HIPAA regulations to ensure patient privacy and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Maintain detailed care plans to track progress, preferences, and specific care requirements for each client
- Conduct patient observation to identify any changes in condition or behavior that may require intervention
- Operate assistive devices such as Hoyer lifts or resident’s
to facilitate safe transfers and mobility - Perform light cleaning duties to uphold a safe and hygienic environment in accordance with health standards
- Document all care activities accurately while adhering to HIPAA guidelines for patient confidentiality
- Collaborate with healthcare teams including nurses, social workers, and family members to coordinate comprehensive care plans
- Provide emotional support and companionship to enhance clients’ quality of life in a respectful manner
Experience
- Prior experience working in assisted living, nursing homes, home health care, or group homes is highly preferred
- Demonstrated knowledge of patient observation techniques and vital signs monitoring
- Experience assisting with ADLs, medication administration, and care planning is advantageous
- Familiarity with dementia care, Alzheimer’s care, memory care, hospice care or working with individuals with developmental disabilities or autism is beneficial
- Certification in CPR and First Aid is required; additional certifications such as HIPAA training or nursing assistant credentials are a plus
- Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary
- Strong communication skills for effective teamwork and client interaction
- Compassionate attitude coupled with patience and resilience to handle diverse caregiving scenarios
