We are seeking a highly motivated Care Transition Coordinator to join our team. In this role, you will work closely with patients and families to ensure a smooth transition from hospital to home or other care settings.
The ideal candidate will have a strong background in nursing and excellent communication skills. You will collaborate with healthcare providers to develop and implement personalized care plans.
- Main Responsibilities:
- Coordinate patient care transitions from hospital to home or other care settings.
- Identify barriers to care and develop strategies to overcome them.
- Collaborate with healthcare providers to ensure that patients receive the necessary support.
Certification:
- Registered Nurse | North Dakota Board of Nursing (NDBON) or Compact State Agreement
- Basic Life Support (BLS) | American Heart Association (AHA)
Requirements:
- A minimum of 3 years of related experience
- Occasional standing, walking, stooping, bending, reaching, squatting, crouching, twisting, handling, seeing, hearing
