General Job Description
- Assess patients’ condition during every visit and chart your observations
- Perform evaluation tasks, including vital signs and medication review
- Administer medication as prescribed by the patient’s Physician
- Dress or redress open wounds and assess progress of healing
- Educate patients and their families on proper home health care strategies and procedures
- Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan
- Provide palliative care as needed to keep the patient comfortable
- Make recommendations for devices or tools that might improve the patient’s quality of life
- Listen to the patient and respond to concerns or requests
- Recertifies patients, Admits new patients , completes Oasis and Plan of Care (RN)
Key skills and qualifications
- Valid LPN or RN license and adherence to state Nurse Practice Acts.
- Strong communication and interpersonal skills to interact with patients, families, and other professionals.
- Ability to work independently and manage time effectively.
- Good organizational skills for managing patient records and schedules.
- Valid driver's license and access to a dependable vehicle are often required for travel between patients' homes.
- Basic Life Support (BLS) certification is typically necessary.
RNs Job Description:
- Patient assessment: Perform head-to-toe assessments, monitor vital signs, and evaluate the patient's physical, functional, and psychosocial status.
- Care planning: Develop, implement, and evaluate individualized nursing care plans based on assessment data, patient needs, and home environment.
- Medication and treatment administration: Administer prescribed medications, including shots and IVs, and ensure patients take the correct medication at the right times.
- Wound care: Clean and redress wounds, assess healing progress, and take preventative measures to minimize infection risk.
- Patient and family education: Teach patients and their families about their diagnosis, treatment plan, medications, and proper home care procedures to promote self-management and safety.
- Health monitoring and documentation: Track patient progress, document observations, and report abnormal findings to the physician.
- Coordination of care: Collaborate with physicians, physical therapists, occupational therapists, and other members of the care team to ensure comprehensive and coordinated care.
Additional duties may include:
- Assist with pain management.
- Provide wound care.
- Ensure proper feeding and hygiene.
- Monitor and operate medical equipment, such as telemonitoring devices.
- Assess the home environment for safety and community resource needs.
- May supervise home health aides & LPNs
LPNs Job Description:
- Patient care: Administer medications and treatments, monitor vital signs (blood pressure, temperature, pulse), and assist with personal care like bathing, dressing, and eating.
- Medical procedures: Perform wound care and dressing changes, collect lab samples, and provide basic medical treatments.
- Patient and family education: Teach patients and their families about their medical conditions, medications, and how to manage their health at home.
- Monitoring and assessment: Conduct patient assessments, observe signs and symptoms of changes in condition, and report them to the appropriate healthcare providers.
- Documentation: Maintain accurate and detailed medical records and document patient progress according to established policies.
- Team collaboration: Communicate with registered nurses, physicians, and other members of the healthcare team about patient needs and status changes.
