What You'll Be Doing

As a Case Manager, you'll be more than a service provider; you'll be a coach, advocate, problem solver, and trusted support person.

You'll:

Help individuals identify goals and create plans for success
Connect people with community resources and support services
Advocate for clients and help remove barriers to independence
Partner with providers, families, and support networks
Support skill building, recovery, and personal growth
Maintain quality documentation and service records
Celebrate victories, big and small, with the people you support

You're a Great Fit If You:

Have a High School Diploma or equivalent
Hold an MHRT-C or MHRT-C Provisional Certification (or are eligible to obtain MHRT-C)
Have experience in behavioral health, mental health, social services, or human services
Have at least one year of experience supporting individuals with mental illness and/or developmental disabilities
Enjoy building relationships and solving problems
Can work independently while also being a great teammate
Have a valid driver's license and reliable transportation
