β€οΈ What You'll Be Doing
As a Case Manager, you'll be more than a service provider; you'll be a coach, advocate, problem solver, and trusted support person.
You'll:
β Help individuals identify goals and create plans for success
β Connect people with community resources and support services
β Advocate for clients and help remove barriers to independence
β Partner with providers, families, and support networks
β Support skill building, recovery, and personal growth
β Maintain quality documentation and service records
β Celebrate victories, big and small, with the people you support
Β
π You're a Great Fit If You:
β Have a High School Diploma or equivalent
β Hold an MHRT-C or MHRT-C Provisional Certification (or are eligible to obtain MHRT-C)
β Have experience in behavioral health, mental health, social services, or human services
β Have at least one year of experience supporting individuals with mental illness and/or developmental disabilities
β Enjoy building relationships and solving problems
β Can work independently while also being a great teammate
β Have a valid driver's license and reliable transportation
