Job Overview
We are seeking a compassionate and dedicated Home Care Aide/Caregiver to provide exceptional in-home support and assistance to individuals with diverse needs. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor and observe patients’ health status, accurately documenting vital signs and changes in condition using Electronic Health Record (EHR) systems.
- Administer medications following medical instructions while maintaining compliance with HIPAA regulations for patient privacy.
- Support clients with specialized needs such as catheter care, hospice care, Alzheimer’s/dementia/memory care, and working with individuals with developmental disabilities.
- Operate medical equipment including Hoyer lifts, and assist with transfers to ensure safety during mobility assistance.
- Prepare nutritious meals and assist with meal planning tailored to dietary restrictions or medical conditions.
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall well-being.
- Collaborate effectively with healthcare teams by documenting medical records and communicating patient progress clearly.
- Provide emotional support and companionship to foster a positive environment for clients living independently or in assisted living facilities.
Qualifications
- Proven experience in home health, senior care, assisted living, nursing homes, or long-term care environments.
- Knowledge of medical terminology, medication administration, vital signs monitoring, and basic nursing procedures.
- Certification in CPR is required; additional training in hospice care or dementia/Alzheimer’s care is preferred.
- Familiarity with EMR/EHR systems for accurate documentation of patient information.
- Experience working with individuals with disabilities or developmental challenges is highly desirable.
- Ability to lift patients safely using equipment such as Hoyer lifts and perform catheter or other specialized medical procedures when necessary.
- Strong communication skills coupled with compassion and patience for providing personalized patient care.
