Coverage depends on patient referrals, therapist availability, and current agency needs.
Job Responsibilities
As a Home Health Physical Therapist, you will:
- Evaluate patients in their homes and develop individualized treatment plans.
- Provide skilled physical therapy services according to physician orders and established plans of care.
- Help patients improve strength, mobility, balance, safety, and independence.
- Provide therapeutic exercises, functional training, and mobility interventions.
- Educate patients, families, and caregivers on exercises, safety, and home programs.
- Monitor patient progress and modify treatment plans as appropriate.
- Communicate with physicians and other members of the interdisciplinary care team.
- Participate in care coordination and case conferences as needed.
- Complete accurate and timely clinical documentation.
- Follow agency policies and applicable state and federal regulations.
Qualifications
- Current Missouri Physical Therapist (PT) license required.
- Valid driver's license and reliable transportation.
- Current auto insurance.
- Ability to travel to patients' homes within your selected coverage area.
- Strong communication, organization, and time-management skills.
- Ability to work independently while collaborating with the care team.
- Home health experience preferred but not required.
