We are seeking a dedicated and experienced Home Care Nurse to join our team. The ideal candidate will possess a strong background in various nursing specialties, including hospital medicine, emergency medicine, and post-acute care. This role offers an opportunity to deliver comprehensive patient care in a home setting, ensuring safety, comfort, and optimal health outcomes for diverse patient populations.
- Provide comprehensive nursing care to patients in their homes, including assessments, medication administration, and vital signs monitoring.
- Perform patient evaluations and develop individualized care plans based on physiological and anatomical knowledge.
- Administer injections, venipuncture, and other medical procedures following proper protocols and HIPAA regulations.
- Utilize electronic health record (EHR) systems to document patient information accurately and maintain detailed medical records.
- Educate patients and families on disease management, medication use, and post-acute care instructions.
- Coordinate with healthcare teams to ensure continuity of care across inpatient, outpatient, clinic, and sub-acute settings.
- Maintain compliance with safety standards such as Basic Life Support (BLS) certification and infection control practices.
- Demonstrate proficiency in handling complex cases involving trauma (including Level I Trauma Center experience), infectious diseases, or emergency situations.
- Valid nursing license with current registration; prior experience in hospital medicine or emergency room settings preferred.
- Proficiency with EMR (Electronic Medical Records) and EHR systems for accurate documentation.
- Knowledge of vital signs assessment, anatomy, physiology, and medication administration protocols.
- Excellent patient assessment skills with attention to detail in complex clinical situations.
- Familiarity with HIPAA regulations ensuring confidentiality of patient information.
