- Provide comprehensive skilled services as ordered by the attending physician
- Educate patients and family members while ensuring patient safety
- Maintain accurate documentation and comply with all legal and regulatory standards
- Current Physical Therapist license in the state of employment
- Minimum of one year of acute care experience required
- Home care experience preferred
- Strong communication and interpersonal skills
- Knowledge of therapy best practices
- Sound clinical judgment and problem-solving ability
- Detail-oriented and observant
- Strong time management and prioritization skills
- Valid driver’s license and auto insurance (listed driver)
- Provide skilled physical therapy in patients’ homes as prescribed
- Monitor, record, and report patient progress and changes
- Perform OASIS assessments and adjust care plans as needed
- Complete initial evaluations within 48–72 hours of referral
- Develop and implement individualized care plans
- Help reduce re-hospitalizations through proactive care
- Deliver therapeutic treatments (exercise, massage, heat, etc.)
- Coordinate outpatient services when necessary
- Ensure continuity of care (admissions, transfers, discharges)
- Educate patients and families on conditions and care plans
- Train patients in the use of assistive equipment
- Ensure awareness of financial responsibilities for equipment
- Supervise PTAs and other support staff as appropriate
- Evaluate and adjust plans of care based on patient response
- Communicate with care team members regarding patient status
- Complete documentation per agency and Medicare guidelines
- Participate in audits, meetings, and training sessions
- Maintain licensure and professional standards
- Travel to patient locations as scheduled
- Maintain daily attendance at assigned visits
- Complete documentation within required timeframes:
- Within 24 hours for routine visits
- Within 48 hours for start-of-care visits
- Submit all required documentation per company standards
- Follow and implement care plans within scope of practice
Health Requirements:
- Must meet all physical job requirements
- Provide proof of negative TB test (or complete testing at orientation)
- Annual TB testing required thereafter
- Must be free of conditions limiting job performance
- Potential exposure to bloodborne pathogens
