Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems.
• Administer medications, injections, IV infusions, and other treatments following established protocols and safety guidelines.
• Monitor patients’ conditions closely, managing acute pain and providing specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments.
• Assist with complex procedures such as catheterization, tube feeding, sterile dressing changes, and ventilator management.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans, discharge planning, and case management strategies.
• Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and patient information.
• Educate patients and their families on health management, medication use, and post-discharge care to promote recovery and independence.

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent credential.
• Proven experience working in diverse healthcare settings such as hospitals (including ICU or trauma centers), nursing homes, skilled nursing facilities, or home care environments.
• Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and clinical procedures such as venipuncture and first aid.
• Experience with EMR/EHR systems like Cerner, Epic, Athenahealth or eClinicalWorks is highly preferred.
• Demonstrated ability to perform patient assessments accurately and manage complex cases involving acute pain or infectious disease care.
• Excellent communication skills for caregiver interaction and client education; experience working with individuals with disabilities or developmental delays is a plus.
• Ability to work effectively in high-pressure environments such as emergency rooms or Level I/II trauma centers; ICU experience is advantageous.
• Knowledge of long-term care regulations and geriatrics-focused practices; experience in hospice or memory care settings is desirable.
• Certification in Basic Life Support (BLS), CPR, sterile technique, catheter care, and other relevant clinical skills required.
