Job Summary:
We are seeking a compassionate and licensed Physical Therapist (PT) to evaluate, diagnose, and treat patients with injuries, illnesses, or disabilities. The ideal candidate will develop individualized treatment plans to improve mobility, reduce pain, restore function, and enhance patients' overall quality of life.
Key Responsibilities
- Perform comprehensive patient evaluations and assessments.
- Develop and implement individualized treatment plans based on patient needs and physician referrals.
- Administer therapeutic exercises, manual therapy, and other rehabilitation techniques.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and their families on treatment goals, home exercise programs, and injury prevention.
- Maintain accurate and timely patient documentation in accordance with regulatory and facility requirements.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
- Ensure compliance with all federal, state, and facility policies and procedures.
- Participate in discharge planning and coordinate ongoing patient care.
