Alter Behavioral Health is an adult mental health treatment provider with a network of upscale inpatient residential facilities in Orange County and San Diego.
- Schedule: Full-Time | Monday through Friday 8:30am - 5pm
- Competitive Compensation: $70,304 - $73,000/year (DOE)
- Location: You will be assigned to one of our Orange County residential facilities located in Dana Point, San Juan Capistrano, or Capistrano Beach.

As a Care Path Manager (Case Manager), your primary focus is managing a caseload of clients and supporting their progress throughout treatment. You will serve as a consistent point of contact, helping ensure each client receives individualized, high-quality care in a structured residential setting.
You play a critical role in shaping both the client experience and the performance of the team supporting them.
What You’ll Do
Case Management
- Manage a caseload of approximately 6 clients in a residential setting
- Serve as the primary point of contact for assigned clients throughout their treatment stay
- Monitor progress toward treatment goals and communicate updates with the clinical team
- Facilitate check-ins, support daily structure, and reinforce therapeutic programming
- Advocate for client needs during treatment planning and interdisciplinary meetings
- Support care coordination, including admissions, transitions, and discharge planning
Client Advocacy
- Represent client needs in treatment planning and interdisciplinary discussions
- Support continuity of care and coordination across services
- Promote adherence to program structure, expectations, and care plans
Crisis Intervention & Safety
- Respond promptly and effectively to behavioral or emotional escalations
- Utilize de-escalation techniques and involve clinical leadership when appropriate
- Ensure a safe, structured, and therapeutic residential environment
Staff Leadership & Oversight
- Support Care Coordinators in daily responsibilities
- Provide real-time feedback, performance guidance, and mentorship
- Assist with staff development
Program Operations & Compliance
- Oversee daily residential operations to ensure efficiency and consistency
- Maintain accurate documentation and support audit readiness
- Ensure compliance with DHCS, DSS, and Joint Commission standards
Program Coordination
- Facilitate groups and support admissions and discharges
- Coordinate communication between departments to ensure continuity of care
- Support implementation of policies, procedures, and best practices

Preferred Qualifications
- CADAC-1 or equivalent certification
- Knowledge of trauma-informed care practices
- Experience working with multidisciplinary teams
- Familiarity with EMR systems
Minimum Qualifications
- High School Diploma required; Bachelor’s or Master’s Degree preferred
- At least one (1) year of experience in behavioral health case management required
- At least one (1) year of leadership or supervisory experience preferred
- Ability to safely and effectively interact with clients experiencing psychiatric symptoms
Requirements
- Valid California Driver’s License required
- CPR & First Aid certification required (or ability to obtain prior to start)
- Must pass Live Scan fingerprint clearance
- Must be legally authorized to work in the United States
Physical Requirements
- Frequent walking, standing, bending, and movement throughout residential facilities
- Ability to navigate multi-level homes, including stairs
- Ability to lift and carry up to 25 pounds
- Ability to safely intervene in crisis situations using trained techniques
- Ability to operate computers and documentation systems
Compliance
Employment is contingent upon:
- Successful Live Scan clearance
- Completion of required health screenings, drug testing, and TB testing
- Compliance with DHCS, DSS, Joint Commission standards, HIPAA, and applicable California employment laws
