Physician Assistant (PA-C) – Post-Acute Care Opportunity

We are seeking a highly skilled and compassionate Physician Assistant (PA-C) to join our team in post-acute care. As a key clinical provider, you will diagnose, treat, and guide patients to better health in a skilled nursing facility.
About the Role

Your Responsibilities:
- Perform physical exams and review medical histories.
- Order and interpret diagnostic tests (labs, imaging, etc.).
- Diagnose and manage acute and chronic conditions.
- Prescribe medications and create treatment plans.
- Collaborate with physicians, nurses, and facility staff.
- Educate patients and families on health conditions and preventive care.
- Document patient care accurately and efficiently.
Requirements:
- PA-C License (or eligibility to obtain)
- All experience levels welcome – training & mentorship available
- Passion for geriatrics, internal medicine, or primary care
- Strong team player with excellent communication skills
- Self-motivated with a patient-first approach
