Job Description
As a Mental Health Professional Case Manager, you will play a vital role in our ACT team. Your primary responsibility will be to work closely with consumers, assessing their needs, and developing plans to address these needs. You will also facilitate linkage to community resources, advocate on behalf of the consumer, and evaluate the consumer's progress towards discharge planning.
Responsibilities
- Assess consumers' needs and develop individualized plans to address these needs.
- Facilitate linkage to community resources, such as healthcare services, housing, and employment opportunities.
- Advocate on behalf of the consumer to secure necessary resources and services.
- Evaluate consumers' progress towards discharge planning and make adjustments as needed.
Requirements
- Bachelor's degree in a mental health discipline.
- Certification in case management and in-home intervention training by the Alabama State Department of Mental Health within six months of employment.
- A valid driver's license and a clear driving record.
- Age 23 or older.
