Responsibilities
- Conduct thorough assessments of patients' wounds and overall health status.
- Develop individualized care plans based on assessment findings and patient needs.
- Perform wound care treatments, including dressing changes, debridement, and infection control measures.
- Educate patients and their families on wound care techniques and preventive measures.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
- Monitor patient progress and adjust treatment plans as necessary.
- Document all assessments, interventions, and outcomes in the medical record accurately.
- Stay updated on best practices in wound management and participate in ongoing education.
Requirements
- Current nursing license (RN or LPN) in the state of practice.
- Experience in Skilled Nursing is preferred.
- Knowledge of medical terminology related to wound care.
- Skills in wound care management are advantageous.
- Strong communication skills to effectively educate patients and collaborate with the healthcare team.
- Ability to work independently as well as part of a team.
