This is a part-time position reporting to the Executive Director, 24 hours/week. The Care Coordinator is responsible for patient-centered case management and for providing program leadership and workflow ownership for the ALS Clinic. This role ensures continuity of care for patients and caregivers, coordinates access to approved care services and oversees the planning and execution of ALS Clinic operations.

This position is non‑supervisory and does not include financial, budgetary, or contracting authority.


KEY RESPONSIBILITIES

Patient Case Management

 

·        Serve as the primary Care Coordinator for approximately 60 ALS patients and their caregivers

·        Provide ongoing support, education, and referrals between clinic visits

·        Conduct routine outreach to assess patient and caregiver needs

·        Coordinate follow‑up care and service access in alignment with approved programs

·        Maintain accurate, timely documentation of patient records and communications in the CRM system


Clinic Program Leadership & Workflow Ownership

(Approximately two clinic days per month, plus preparation and follow‑up)


·        Provide program leadership for ALS Clinic operations, ensuring clinic workflows are executed consistently and effectively

·        Own and manage clinic scheduling, including scheduling patients for clinic appointments

·        Coordinate clinic calendars and prepare clinic schedules and materials

·        Serve as the primary point of contact for patients and families regarding clinic participation

·        Coordinate with the multidisciplinary clinic team to ensure readiness for clinic days

·        Lead clinic‑day workflow and ensure timely post‑clinic follow‑up and documentation

·        Identify workflow or operational improvement opportunities and present recommendations to the Executive Director


Care Services Coordination (Execution‑Focused)

 

·        Facilitate patient access to approved ALS United RI care programs (subject to annual changes)

·        Distribute applications, collect required documentation, and prepare materials for review

·        Coordinate logistics with approved vendors as directed

·        Track program utilization and provide data or reports as requested


Support Groups & Education

 

·        Plan and facilitate one (1) patient or caregiver community group per month

·        Prepare materials and coordinate logistics for the monthly group

·        Assist with care‑related educational programming as assigned

·        Share approved program information for clinic days and internal communications


Administrative & Collaboration


·        Maintain shared documentation and tracking tools

·        Monitor and respond to assigned support communications

·        Collaborate closely with internal staff and clinic partners

·        Participate in organizational and clinic‑related meetings as required


Role Scope

To ensure clarity and organizational alignment:


This role does not manage or oversee the Equipment Loan Program

This role does not supervise staff, volunteers, or vendors

This role frequently collaborates with the Programs Assistant and Office Manager and external vendors.

Program changes with financial, staffing, or contractual impact require approval from the Executive Director.


PREFERRED QUALIFICATIONS

·        Bachelor’s degree in social work, human services, or related field preferred

·        Experience supporting individuals with chronic or terminal illness strongly preferred

·        Strong organizational, communication, and documentation skills

·        Ability to work collaboratively within a defined scope of responsibility

·        Reliable transportation required for clinic days and in‑person duties
