Medical Social Worker – Home Health
Alpine Home Health Services is looking for a compassionate and dependable Medical Social Worker to join our growing home health team.
This is a flexible position and a great opportunity for someone looking for PRN, part-time, or supplemental work with a supportive agency.
What You’ll Do:
- Complete psychosocial assessments for home health patients
- Help patients and families connect with community resources
- Assist with transportation, financial, housing, caregiver, and long-term planning needs
- Provide supportive counseling and education
- Coordinate with nurses, therapists, physicians, and caregivers
- Complete timely documentation in the EMR
What We’re Looking For:
- Social work degree required
- Experience in home health, hospice, hospital, SNF, or geriatric care preferred
- Familiarity with Medicare/home health is a plus
- Strong communication and documentation skills
- Reliable, compassionate, and comfortable working independently in patients’ homes
