Job Summary
We are seeking a Licensed Practical Nurse (LPN) to deliver compassionate, patient-centered nursing care within the home setting. This role involves assessing and observing patients and/or caregivers to enhance their quality of life and providing individualized education. The LPN will adhere to nursing policies and procedures, follow physician-directed care plans, and uphold Medicare and Medicaid regulations to ensure high standards of patient care.
Qualifications/Educational Requirements
- Current Licensed Practical Nurse (LPN) licensure.
- Minimum of 1 year of experience in a healthcare setting.
- Strong oral and written communication skills.
Key Responsibilities
- Collaborate effectively with the healthcare team to ensure seamless patient care.
- Utilize strong organizational and time-management skills.
- Support quality improvement initiatives and strive to uphold agency standards.
- Conduct nursing procedures in alignment with agency protocols.
- Provide care under the supervision of a Registered Nurse (RN).
- Monitor and document patient reactions and progress through observation, assessment, and evaluation.
- Educate patients and family members on disease management, medications, treatment options, and home care procedures as outlined in the plan of care.
- Report any adverse findings promptly to the physician and RN.
- Follow Medicare and Medicaid guidelines for patient care delivery.
- Coordinate and oversee patient care services to meet individual needs.
- Comply with HIPAA regulations and ensure confidentiality both inside and outside the office.
- Adhere to infection control protocols in all patient interactions.
- Document each skilled visit according to Medicare standards.
- Maintain accurate patient records following agency policies and procedures.
- Participate in ongoing training, in-services, and professional development annually.
- Perform other duties as assigned, supporting overall patient care and agency goals.
- Ability to lift up to 50 lbs.
Hourly Range: $40.00 - $45.00, commensurate with experience and qualifications.
