Physical Therapist

Role: A compassionate and motivated Physical Therapist responsible for evaluating, developing, and implementing individualized treatment plans to help patients restore function, improve mobility, reduce pain, and maximize independence.

Key Responsibilities

• Assess patients and create personalized treatment strategies.
• Deliver skilled physical therapy using evidence-based techniques.
• Monitor progress and adjust treatment plans accordingly.
• Educate patients and caregivers on exercises, safety, and assistive devices.
• Collaborate with physicians and multidisciplinary teams to enhance patient outcomes.
• Complete accurate clinical documentation and participate in discharge planning.
• Mentor assistants, students, and support staff, and engage in quality improvement and continuing education initiatives.
• Ensure compliance with HIPAA, accreditation standards, and organizational policies.
  
  

Qualifications & Skills

• Graduate of an accredited Physical Therapy program (DPT preferred).
• Current or eligible Illinois Physical Therapist license.
• CPR/BLS certification or ability to obtain.
• One to two years of clinical experience preferred; new graduates encouraged to apply.
• Strong assessment, communication, and critical thinking skills.
• Ability to work effectively within a team and deliver compassionate, patient-centered care.
