Part Time RN Care Manager

Role Overview: The Care Manager plays a key role in facilitating seamless discharge planning from admission to discharge, collaborating with the healthcare team to ensure quality patient care, safety, and efficient throughput.

Responsibilities

• Coordinate discharge processes and intervene with patients having complex or high-risk psychosocial needs.
• Promote a coordinated care model that integrates clinical, safety, and financial components.
• Support performance optimization by adopting best practices and promoting team collaboration.
  
  

Qualifications & Skills

• Graduate of an accredited School of Nursing and holds a current state license.
• Bachelor’s Degree of Science in Nursing required.
• 2-3 years of experience in an acute care hospital setting preferred.
• Knowledge of care guidelines such as InterQual or Milliman is advantageous.
• Strong communication, assessment, critical thinking, and computer skills.
• Certification in Basic Life Support (BLS) required; Case Management certification preferred.
