Occupational Therapist - Home Health

Role Overview: This position involves providing medically prescribed occupational therapy treatments to restore function and prevent disability following injury. The therapist collaborates with healthcare teams to deliver quality, holistic, patient-centered care in home settings, supporting the organization's mission of care, comfort, and healing.

Key Responsibilities

• Assess patient needs and develop comprehensive care plans.
• Implement occupational therapy techniques using appropriate principles, methods, materials, and equipment.
• Work as part of an integrated healthcare team to ensure coordinated patient care.
• Travel within the service area to patients' homes as needed.
• Demonstrate professional procedures in a home care environment.
  
  

Qualifications & Skills

• Graduate of an accredited Occupational Therapy Assistant program.
• Current state license and valid driver's license.
• CPR certification.
• Excellent communication and interpersonal skills.
• Proficiency with office equipment, including tablets and cell phones.
• Ability to work flexible hours, including weekends and holidays.
