LVN/LPN (Part-Time or Full-Time)

Role: Licensed Practical/Vocational Nurse responsible for delivering direct patient care under supervision, following the care plan, providing treatments, and collaborating with team members to achieve positive patient outcomes.

Key Responsibilities

• Provide direct patient care according to state Nurse Practice Act.
• Implement and document care plans accurately and timely.
• Assess patients and educate patients and families about diagnoses and care.
• Coordinate home health services and report needs to registered nurses or managers.
• Utilize equipment efficiently and participate in professional growth.
• Communicate effectively with team members, document clinical notes, and report concerns.
  
  

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Minimum of three years nursing experience, including at least one year in home health or medical/surgical settings.
• Current license as an LPN/VN in the state.
• Good communication, organization, and documentation skills.
• Valid driver’s license with insured, operational vehicle.
• Current CPR certification.

• Physical ability to walk, stand, lift, and perform necessary nursing tasks requiring visual and auditory acuity.
