- Provide direct patient care according to state Nurse Practice Act.
- Implement and document care plans accurately and timely.
- Assess patients and educate patients and families about diagnoses and care.
- Coordinate home health services and report needs to registered nurses or managers.
- Utilize equipment efficiently and participate in professional growth.
- Communicate effectively with team members, document clinical notes, and report concerns.
- Graduate of an accredited practical or vocational nursing program.
- Minimum of three years nursing experience, including at least one year in home health or medical/surgical settings.
- Current license as an LPN/VN in the state.
- Good communication, organization, and documentation skills.
- Valid driver’s license with insured, operational vehicle.
- Current CPR certification.
- Self-directed with minimal supervision.
- Strong observation, problem-solving, and communication skills.
- Physical ability to walk, stand, lift, and perform necessary nursing tasks requiring visual and auditory acuity.
