Collaborate with the healthcare team, document patient visits thoroughly, and participate in care coordination activities to ensure high-quality patient care. Work under supervision of a Registered Nurse or designated supervisor.

Responsibilities
- Deliver clinical services within the scope of practice, adhering to state laws and infection control procedures.
- Accurately document patient visits in electronic medical records and visit verification systems.
- Participate in care coordination by reporting patient needs and condition changes, attending case conferences, and communicating effectively with patients, families, and healthcare providers.
- Current LPN licensure and CPR certification.
- Valid driver’s license, vehicle insurance, or reliable transportation.
- Ability to work effectively in diverse home settings and manage multiple tasks independently.
- Strong communication, organizational, and documentation skills.
- Preferably, home care experience and flexibility to meet client needs.
