Role: Licensed Vocational Nurse (LVN)/LPN in a home health setting, providing direct patient care to pediatric, adolescent, adult, and geriatric patients.
Responsibilities: Under RN supervision, perform technical nursing functions, document patient progress, administer medications and treatments, maintain a safe environment, identify abnormalities, re-evaluate nursing needs, counsel patients and families, and coordinate services with the care team.
Qualifications & Skills: Graduate of an accredited nursing program, current LPN/LVN license, at least one year of experience, valid CPR certification, valid driver’s license, and ability to travel. Must demonstrate strong documentation, communication, and assessment skills.
