- Administering medications, wound care, and skilled nursing procedures.
- Assisting residents with activities of daily living, ensuring their comfort, dignity, and safety.
- Supervising nursing assistants and medication aides during shifts.
- Monitoring residents' physical and emotional well-being and notifying physicians of any changes.
- Developing, implementing, and reviewing individualized care plans in collaboration with the interdisciplinary team.
- Completing accurate clinical documentation and coordinating admissions, transfers, and discharges.
- Participating in quality assurance, staff education, and regulatory activities.
- Performing additional duties as assigned by leadership.
- Active LPN/LVN license, CPR/BLS certification preferred.
- Minimum one year of clinical nursing experience; long-term care and supervisory experience preferred.
- Strong clinical, observation, medication, and documentation skills.
- Knowledge of long-term care regulations and proficiency with electronic health records and Microsoft Office.
- Excellent communication, independent working ability, and sound clinical judgment.
