Home Health Physical Therapist

Role Overview: A Home Health Physical Therapist provides personalized physical therapy services in patients' homes to enhance mobility, strength, and independence. Reporting to the Clinical Manager, you will develop and implement treatment plans, assess patient progress, and document interventions to support recovery and functional improvement.

Key Responsibilities

• Plan and deliver prescribed physical therapy treatments onsite in patients' homes.
• Assess and document patients' physical capabilities to inform treatment goals.
• Create and execute rehabilitation programs aligned with physician plans and patient needs.
• Provide clear instructions to PTAs and Home Health Aides, overseeing their care and documentation.
• Maintain thorough records of patient evaluations, interventions, and progress reports.
• Communicate patient progress to physicians, care teams, patients, and families.
  
  

Qualifications & Skills

• Current, unrestricted Physical Therapy license.
• CPR certification.
• Experience collaborating within healthcare teams.
  
  

Preferred: One year of clinical experience as a Physical Therapist.

Additional Details: Compensation varies based on location and experience, with a pay range of $93,000 - $128,000 annually. The role involves patient-facing responsibilities, requiring TB screening and a valid driver’s license with personal vehicle insurance.
