Home Health Physical Therapist (PT) Reports To: Clinical Manager / Director of Rehabilitation Position Summary The Home Health Physical Therapist provides skilled, patient-centered therapy services to individuals in their homes to restore function, improve mobility, reduce pain, and prevent disability following illness, injury, or hospitalization. This role combines clinical expertise with autonomy, care coordination, and compassionate patient engagement to support safe recovery and independence in the home environment. The PT collaborates closely with an interdisciplinary care team to deliver evidence-based rehabilitation aligned with physician orders, agency standards, and state and federal home health regulations. Key Responsibilities Patient Care & Clinical Practice Perform comprehensive physical therapy evaluations in the home setting, assessing functional mobility, strength, balance, gait, safety risks, and environmental factors. Develop individualized treatment plans with measurable goals based on physician orders and patient needs. Provide skilled therapeutic interventions including: Therapeutic exercise Gait and balance training Neuromuscular re-education Transfer and mobility training Pain management strategies Fall prevention education Assess home safety and recommend adaptive equipment or modifications to promote independence. Reassess patient progress and adjust treatment plans accordingly. Care Coordination Collaborate with nurses, occupational therapists, speech therapists, social workers, and physicians to ensure coordinated care. Participate in interdisciplinary case conferences and care planning. Educate patients, families, and caregivers on diagnosis, prognosis, and home exercise programs. Communicate changes in patient condition promptly to appropriate team members. Documentation & Compliance Complete timely and accurate electronic documentation in accordance with agency policy and Medicare home health regulations. Maintain compliance with OASIS documentation requirements and quality standards. Support agency quality initiatives and outcome improvement efforts. Patient Experience & Advocacy Foster trusting therapeutic relationships that support patient confidence and engagement. Promote patient dignity, autonomy, and culturally responsive care. Encourage self-management strategies that support long-term wellness and independence. Qualifications Required Licensed Physical Therapist in the State of Washington. Graduate of an accredited Physical Therapy program (DPT or equivalent). Current BLS certification. Valid driver’s license, reliable transportation, and ability to travel throughout the service area. Strong clinical assessment and care planning skills. Ability to work independently in a home-based setting. Preferred Minimum one (1) year of clinical experience; home health experience strongly preferred. Experience with OASIS documentation. Knowledge of Medicare Conditions of Participation for Home Health. Experience with geriatric and medically complex populations.
