Role Overview: The Home Health Occupational Therapist Assistant provides personalized therapy services to help patients regain strength, mobility, and independence in their homes. They work under the supervision of a qualified Occupational Therapist, implementing treatment plans, training patients and families in daily skills, documenting progress, and participating in care coordination and discharge planning.

- Deliver therapy and educational activities based on the patient's Plan of Treatment
- Train patients and families in activities such as dressing, feeding, grooming, and hygiene
- Maintain accurate documentation of patient progress and clinical notes
- Collaborate with healthcare team members to meet patient needs and plan discharges
- Current and unrestricted Occupational Therapy Assistant license in North Carolina
- Valid driver’s license, auto insurance, and reliable transportation
- Current CPR certification
- Preferred: Home health experience
