Job Summary
The Case Manager plays a critical role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized service plans, and connecting clients with necessary resources. The ideal candidate will possess a strong background in human services, case management, or possess an associates degree in nursing (RN license), and demonstrate a commitment to improving the lives of those they serve.

Duties

• Conduct assessments to identify client needs and develop tailored service plans.
• Provide ongoing support and guidance to clients.
• Facilitate access to resources related to daily living activities.
• Foster client personal growth and independence.
• Supervise and coordinate services provided by other professionals involved in the client's care.
• Maintain accurate documentation of client interactions, progress notes, and service outcomes.
• Collaborate with community organizations to enhance service delivery.

Qualifications

• Bachelor's degree in a related field preferred; relevant experience may be considered in lieu of degree.
• Experience in case management, human services, or nursing (RN licensure) is highly desirable.
• Knowledge of case management, long term care, community based services.
• Strong management skills with the ability to effectively manage a caseload of 50 plus clients.
• Excellent communication and interpersonal skills for building rapport with clients and collaborating with team members.
• Ability to work independently as well as part of a multidisciplinary team.
• Commitment to maintaining confidentiality and adhering to ethical standards in case management.
